Appendicitis
De blindedarm of appendix is een blinde zak in de vorm van een vinger die in verbinding staat met de dikke darm. Ten gevolge van een infectie zwelt deze op en vult zich dan met pus, en wordt dan appendicitis genoemd.
Als een blindedarmontsteking niet op tijd wordt behandeld, kan de appendix perforeren en kan er pus in de buikholte lekken. Als de pus zich collecteert op één plek, wordt het een abces genoemd. Als de pus in de hele buikholte lekt, spreekt men van een diffuse peritonitis.
De symptomen kunnen variëren in functie van de lokalisatie en de lengte van de blindedarm.
Acute blindedarmontsteking kan mensen van alle leeftijden treffen.
Het komt vooral voor tussen de leeftijd van 10 en 30 jaar. Het is zeer zeldzaam voor de leeftijd van 2 jaar.
De diagnose van appendicitis kan moeilijk zijn, omdat de symptomen aspecifiek kunnen zijn.
De symptomen zijn afhankelijk van de lokalisatie van de appendix in de buikholte.
Bij zwangere vrouwen kan de pijn zich voornamelijk in de bovenbuik bevinden, doordat de blindedarm onder druk van de foetus naar boven in de buikholte geduwd wordt.
1. Infectie van de wand van de appendix.
Als gevolg van de infectie zwelt de wand op en wordt het blindedarmkanaal afgesloten, waardoor de bacteriën in de blindedarm niet meer kunnen afvloeien. Bacteriën die opgesloten zitten in de appendix vermenigvuldigen zich, vormen een abces en verdere ontsteking van de appendix.
2. Obstructie van het kanaal van de appendix
Als gevolg van een harde ontlasting die het appendix kanaal afsluit, wordt de evacuatie van bacteriën uit de appendix naar de darm verhindert. Bacteriën die opgesloten zitten in de blindedarm vermenigvuldigen zich, creëren een abces en verdere ontsteking van de blindedarm.
Neen, appendicitis kan niet worden voorkomen.
Bij de foetus worden er hormonen geproduceerd die helpen om zijn ontwikkeling te bevorderen.
Bij volwassenen speelt de blindedarm een rol in de immuniteit. Het lymfoide weefsel in de appendix neemt toe tot het tweede en derde decennium, waarna het afneemt en op de leeftijd van 60 jaar helemaal verdwijnt.
Ja, mensen bij wie de blindedarm is verwijderd, hebben dezelfde levensverwachting en levenskwaliteit als mensen die niet zijn geopereerd.
Wanneer de diagnose enkele dagen na het begin van de infectie van de appendix wordt gesteld, kan de ontsteking zo ernstig zijn dat een operatie in de acute fase niet meer wenselijk is.
Als gevolg van de ontsteking die reeds verscheidene dagen aanhoudt, zijn de weefsels rond de geïnfecteerde appendix namelijk sterk oedemateus (gezwollen) en plakken zij zich vast aan de blindedarm om de plaatselijke infectie in te dijken. In dat stadium is het moeilijk om de verschillende samen geplakte structuren en organen in de buikholte te onderscheiden. De dissectie van de vergroeide weefsels kan leiden tot perforatie van organen (bv dikke en dunne darmen) en ernstige bloedingen.
Het risico op complicaties van de operatie wordt dan groter dan het risico op behandeling van de appendicitis met antibiotica. In deze gevallen wordt de appendix behandeld met antibiotica en zo nodig met (percutane of chirurgische) drainage van het abces zonder de appendix te verwijderen.
Na minimaal vier tot zes weken wordt de appendicectomie uitgevoerd. Indien de patiënt ouder is dan 50 jaar of indien er een vermoeden bestaat van een andere pathologie in de appendix, wordt een colonoscopie verricht om de aanwezigheid van een tumor van de darm uit te sluiten vooraleer de appendix te verwijderen.

Een appendectomie is het verwijderen van de blindedarm, een orgaan aan het begin van de dikke darm dat een rol speelt bij de immuniteit.
